Webinar: On-site and Off-site Health Care Team Collaboration
February 29 @ 1:00 pm - 2:00 pm
Coordination of care is challenging within most facilities. When patients need additional resources from off-site providers, strong strategies and processes are critical to making sure care is coordinated, patient diagnoses and treatment instructions are documented, and follow-up care is provided. This webinar will discuss general principles of coordination of care and patient hand-offs, strategies for effective information sharing, best practices for establishing strong relationships with off-site specialists, and the role of medical records in care coordination. Roles of team members, setting expectations, and patient consent for records release will be highlighted. Be prepared with strategies and techniques to make sure your patients receive appropriate care.
