If you are a conference participant and wish to receive continuing
education credit, you will need to submit the following:
Continuing Education Registration Form. Record your participation in
educational sessions using the continuing education registration
form, which is enclosed with the packet you receive when you
check in at the registration center. You may claim only those
hours spent in educational programs. Conference Evaluation Form. Your conference
materials include a conference evaluation form. Use this form to provide the
education
committee with important information to help us plan future
educational programs. Submit these items on site when the
conference ends (or when you leave) and receive your certificate of
participation immediately. If you prefer, you may send the materials
to NCCHC and a certificate will be mailed to you in 6 to 8 weeks.
