Continuing Certification - Guidelines

Guidelines for Application for Continuing Certification

Introduction
To maintain your certification, you must submit annually an application for continuing certification. The purpose of the application is to verify your contact information and make sure that you meet the recertification requirements.

Eligibility
CCHPs must be of good character and professional reputation, have no legal or ethical impediment to providing service in the correctional health care field and have credentials that are free of any restriction that would limit their professional practice solely to the correctional setting.

Application Procedures

  1. Application: The data that you provide on the application for continuing certification will be entered in your CCHP file automatically. Complete all appropriate sections of the form. Be certain to read the instructions carefully.

  2. Recertification Statement: Read and sign the recertification statement on the application.

  3. Continuing Education: Recertification policies require your participation in at least 18 contact hours of continuing education activities. There are two categories of continuing education activities. (See the frequently asked questions for a definition of each category.)
    Provide a list continuing education activities, including dates, titles and number of hours, on page 2 of the application for continuing certification. Note: Documentation of each continuing education activity is no longer required. However, all information supplied on your application for continuing certification is subject to audit by the CCHP Board of Trustees and staff. It is highly recommended that you maintain documentation of your continuing education activities in the event your file comes under such review.

  4. Recertification Fee: A fee of $75 must accompany your application. Payment is accepted in the form of a check, money order or credit card (MasterCard, Visa or American Express). Purchase orders are not accepted. If the application is submitted after the deadline, a $25 late fee must be added.

  5. Application Deadline: Depending on which cycle you are in, recertification materials are due either June 30 or December 31.

  6. CCHP Certificate: When all of the above requirements have been fulfilled, you will receive an updated certificate that expires in one year. Due to the heavy volume of applications received, please allow four weeks for processing of your application and mailing of updated certification documents.
